Doctor's Patient Referral Form Instructions
If you have a patient referral, please download and use our doctor's referral form (in PDF). Once downloaded, you may print it and fax it to us as in the past. Unlike other normal PDFs, this form is ALSO fillable and emailable, meaning that you may open it up and fill-in the blanks on your computer and send it all at one sitting. Once filled out, you may want to save the resultant file as the patient's name (for your records).
The PDF itself has 'email' buttons along the bottom of the form that will automagically open your resident email program, attach the completed form and send it directly to the selected CTMAX office, all in one operation.
If you've already completed and saved the PDF to your hard drive (or you have other documents you wish to send), all you need do is click one of the links below, attach your document(s) and hit send. Each office has its own general email address and clicking one of the links below will open your resident email program, properly address it and enter a subject line automagically.
Alternatively OR if you also have pertinent x-ray images (often too large to email) which you want to provide us, please use our online dropbox (link below) which allows files up to 2Gb to be sent to us. With the dropbox, you may drop any/all documents, pdfs, images, xrays and all will be received securely. CTMAX staff will ensure the files get to the correct office location.
