Please use the form below to safely transmit documents (patient history, insurance forms, referral documents, etc. and/or large files (scans, photos, X-rays, etc.) to us.
You may send multiple files by clicking on "Add another file", but the maximum total upload size is 2Gb. If you have many files or a single really large file, you may want to consider ZIPPING them to group them together and/or reduce the overall file size to be under the 2Gb limit and reduce the transmit time.
The files are securely stored in a place only we may access. We are notified immediately upon a successful upload so we may review them. All files are automatically purged after 14 days.
